Limited License – Application – July 1, 2015
New 15 day license for SLP’s or AuD’s who will be using clients/patients for demonstration purposes at a conference, seminar or other educational setting.
Licensing Fee Schedule – Effective July 1, 2015
Fees are effective July 1, 2015 as approved by Board action on June 27, 2015.
Hearing Aid Board renewal fees remain in affect for the current renewal period including the late renewal fee of $100 if license is not renewed by June 30, 2015.

Reinstatement Application and Instructions
If your license has been expired more than 30 days but less than 3 years, you are eligible for license reinstatement. Please read instructions as additional documentation is required.
Complaint Form, Instructions and Medical Release Form
If you believe a violation of the law or regulations has or may occur, report those actions by completing the Complaint Form. A medical release is required to obtain confidential client/patient information.
